Law Enforcement Holder Forms

Electronic reporting is required for all unclaimed stolen property law enforcement holders. However, the State Treasurer may waive the electronic filing requirements and allow paper reporting if the holder demonstrates that a hardship would result if required to file electronically.

To qualify for the waiver, a written request containing the following information must be submitted:

  1. The name, federal tax identification number and mailing address of the holder
  2. The report year for which the waiver is requested
  3. A detailed statement which lists:
    1. Why the holder cannot submit electronically
    2. The steps the holder is staking to meet the electronic filing requirements
    3. The hardship that would result, including incremental costs that are above and beyond the costs to file on paper
    4. A statement as to what steps the holder will take to assure the ability to file electronically for the next reporting period
    5. The statement (signed by a legal representative of the holder): “Under penalties of perjury, I declare that the information contained in this waiver request is true, correct and complete to the best of my knowledge and belief.”

The waiver request must be filed by Apr. 15 for life insurance companies and by Oct. 15 for all other holders.
